What is a tone poem?
In the early 40's to 50's composers began composing particular musical " ideas" describing places, feelings, events, etc. in one continuous music movement, stepping away from the traditional form of Intro, development, recapitulation forms of classic styles. Composers may label any of their works tonepoems and so do I.
